Agreement and first steps

Initial consultation and assessment

We begin with a personal consultation to get to know you and your loved one. This can take place in your current home, hospital, or right here at our care home — whatever works best for your situation. During this visit, we’ll assess care needs, preferences, and routines so we can provide the right level of support.

Customized care plan and agreement

Based on the assessment, we create a tailored care plan that addresses daily needs, medical requirements, and lifestyle preferences. Once everything is clear and all questions are answered, we finalize an agreement so you know exactly what to expect.

Preparing for move-in

Coordinating the details

Our Care Coordinator helps you plan the move-in date, arrange transportation if needed, and take care of the logistics so the process feels manageable.

Making it feel like home

Our bedrooms are unfurnished. We encourage families to bring favorite furniture, personal touches, and décor to make the space truly their own.

Records and supplies

We’ll guide you through gathering medical records, prescriptions, and any equipment your loved one may need. Our team coordinates delivery and setup so everything is ready on arrival.

Move-in day

A warm welcome

On the big day, our staff is there to greet you and your loved one with care and kindness. We know moving can feel overwhelming, and we aim to make the transition as smooth as possible.

Orientation and introductions

Your loved one will meet caregivers, tour the home, and get familiar with daily routines. We’ll also introduce them to fellow residents, helping new friendships form right away.

Settling in

From unpacking belongings to adjusting to a new routine, our caregivers provide hands-on support so your loved one feels comfortable and cared for immediately.

Ongoing care and communication

Monthly check-ins

We regularly review each resident’s care plan and adjust as needs evolve. Families receive consistent updates on well-being, daily activities, and any changes to care.

Open communication

We believe families are an essential part of the care team. Our Care Coordinator and staff are always available for questions, updates, or conversations about your loved one’s needs.

Continuous improvement

We welcome your feedback and work hard to continuously improve our care. Regular reviews and adjustments ensure your loved one always receives the highest quality support.
